Through a network of community offices throughout the City and County of Peterborough, our staff and volunteers provide a variety of essential services to seniors (60+) and to adults (18+) living with a disability.

REQUEST FOR SERVICES

If you are interested in any of our services for yourself or a family member, please request services here.

You can also contact any Community Care office to register for our services. Many people are referred to us by their physician, another health professional, a family member, or a friend who is familiar with us. Registration is required for most services, except seminars and workshops.

Eligibility for services is based upon client service criteria, assessed need, and priority setting. Services that are provided are intended to help the people receiving support maintain their independence in the community.

Once we have received your information, a staff member from Community Care will contact you to discuss our services with you, either over the phone or in person. They will help you understand how the services will be delivered to you, and go over any fees that may apply.
